However, he discovered unlike his previous Toyota Highlander, the 2023 Lexus actually uses lug bolts (not sure if this is the right terminology) vs. lug nuts. The guy doing his tires noted the OEM lug bolts that go onto the 21" OEM rim are conical in shape where as the after market ones require flat surface lug bolts.
What do you guys recommend as far as lug bolts are concerned? Is this something that can be bought just on Amazon? Are all lug bolt threads the same while the head can be different sizes?
I'd be asking the wheel supplier for the lug bolts that they recommend. They can provide (or recommend a vendor).
You need to use the correct thread size, and length to match the thickness of the wheel hubs. SOME aftermarket wheels are finicky about having washers under the bolt heads. And, bolt torque might be different. <--- lots to contemplate!
In the past, it was not uncommon to need different lug nuts or bolts when winter 'steelie' rims were fitted...needed those for the steel winter wheels with our old Infiniti, 20 years ago.

Thanks! My dad and I are heading back to the tire shop tomorrow to see if they have something that'll work with the RX. Just out of curiosity, did the previous RXs have studs and lug nuts like your basic Toyota?

Since they are lug bolts now, center bore/hub diameter matters as well. You want to avoid hub centric rings if you can. If what I found on the internet is correct, the hub bore is 60.1 mm .
I looked into this in great detail. Spoke to a guy at Tire Rack and he said a 235/R55/20 wheel will work fine and clear the 6 piston rotors. I spoke to two separate Lexus dealers (Spoke to the service managers). Their concern is the sensors will not work properly with aftermarket wheels because of not fitting properly. Lexus does not have a 20" wheel for the 500H. I am going to call Tire Rack back and see if they will take the wheels back if everything doesn't work as it should.
OK so after going back to the tire shop to return the original rims/tires, this is what happened:
Firstly, my dad's RX is the 350h with the 21" pacakge.
The rims we ended up going with are the RS DR3, size 19x8, 5 x 114.3, gloss black which uses the factory lug bolts. 19" was minimum size required to clear the calipers.
Tire wise, we went with Michelin X-Ice snow 107H, size 235-60-19
The tire shop also included hub rings for proper fitment
Unfortunately, they had one of the wheels installed backwards so I need to go back Tuesday to get it flipped. I'll install the tires on my dad's RX after and post pics.
Yes, the operative words are "decent set of tires" - All weathers like Nokians are great. But if I lived in Revelstoke (BC) or Tahoe, I would run studded winters.

Hopefully, one of the tire makers like Nokian will release an All-Season winter rated soon, in our size.
