I use Spotify over wireless Android Auto in my 22 350h and pretty much every drive I take, even if it's just 20 minutes, my music will cut out at least once at some point in the drive (many times on longer drives). AA seems to stay connected the whole time (maps continues updating on the screen), but the music goes silent even though Spotify appears to continue playing. To get the sound back I have to cycle through the car's audio sources (FM/AM/Amazon/XM) and when I get back to AA the audio will return. I'm happy to buy a new phone if I'm the only one with this issue and a new phone will fix it.
On 12/14/22 picked up a 23 NX 350h. Wife at the time had a Samsung S9 about four years old. I have a S21. I had no problems with my phone. She is the primary driver and her S9 had all sorts of problem of not connecting to Android Auto or even bluetooth or freezing up. We just bought her a S22 and not one problem.
You're not alone, it happens to me as well. I'm not sure but I think it could be a wireless Android Auto coupled with Lexus interface issue. I keep forgetting to plug in my phone to use AA each time I get in the car to test it out. Not sure what phone you have but I noticed the person above also had issues with an older Samsung and I currently have a S22 ultra and I have issues from time to time. So it could also be a Samsung issue.

You may be victim of 5G. While 5G supports much higher data transfer speeds it has one big limitation - short range. 5G has much shorter range than 4G, requiring many more transfers from one antenna to another. This transfer not always go smoothly. This does not affect navigation because Google maps buffers the entire route and as long as you don't divert from that route, Google does not need to reconnect to the Internet.
Another common issue may be an app running in the background that also uses Bluetooth. For example, phone or FB messenger. These apps can interrupt other audio transfer like Spotify. A common issue I had with another phone was that if I received a call while playing music, the audio would interrupt for about half a second every 15-20 seconds after hanging up. Basically, the phone app would never fully release the Bluetooth connection and was regularly interrupting the music transfer. The only way to fix it was to close all apps and restart Spotify. Eventually the issue was fixed with an update to the operating system.

However, I'm having connection issues through the standard Lexus interface without Android Auto. When I'm calling, the call will go to the handset and not the system Bluetooth. The phone is newish (1 year old pixel 6pro). You think it's other software interference on the phone?

I don't think it's your phone's problem. It's just that Toyota / Lexus doesn't keep up with the software evolution. All my Toyotas (Venza, RAV4, Highlander, Lexus NX) have/had this issue. They work fine for a year or two with older phones, and then when I replace the old phone with the latest model or simply update the software, problems start.
Some car manufacturers are pretty good with keeping their software up to date (Tesla, Stellantis, Hyundai/KIA, Mercedes). Japanese manufacturers keep living on the past. Did you know that fax is still #1 choice to order takeaway food from restaurants in Japan? Apparently the Japanese still live in the 1990s Internet.
Don't have NX but my 2022 ES has the same issue.
It is not the internet (cutting out the music) as I have locally downloaded music to my phone and it still does it..
My car doesn't have wireless Android Auto so I am always wired and it does it..
I used this phone on different cars and it doesn't have this issue.
So clearly something in Lexus Android Auto integration causing this issue.
No issues here, S23 and stream Stingray music 100% with Waze up all the time, just my default for both mine and Wifes' setup. Get calls, texts and all stay connected. Have the ML system. Am on a 4G plan, though, so that may make a difference as explained above.

It is nothing to do with the connection.
Android Auto does not stop working when it happens. It is only the audio cuts out for half a second randomly.
I can still see the Android Auto screen when the audio cuts out.
If the connection is lost, my screen returns to factory navigation screen.

Try clearing cache in Android Auto and Bluetooth. This helps with a lot of connectivity problems. S 21 S 22 RX and NX and zero problems.
