Toyota/Lexus does not recommend changing the fuel filter as a maintance item. I had a 95 Landcruiser with 130K miles on it and never changed the fuel filter asked the dealer and they said they never changed them. Don't worry be happy

It's in the fuel tank with the fuel pump.
Serviced and replaced with the pump.

The '89-00 LS4 the fuel filter is accessible on the left rear just above the half-shaft. On my service schedule, I recollect it was a 60K mile interval. I changed mine at the time and complete cost was ~$110.

In many it is indeed in the tank, and is something like a nylon sock of sorts.

It depends on the vehicle, some are external.
