Ok, a bit of an update. If I walk up to the door with the key in my hand it will unlock from the outside. But if I hit the unlock button from the driver seat it wont unlock.
Does this still sound like an actuator?
I guess I am not understanding why it would unlock from the outside by touching the handle, but not inside.
If it unlocks from the passenger door switch at will, then it sounds like your door switch on the driver's door is shot. My gut still goes with the actuator and you are catching it working intermittently.

I need to try the passenger side switch when my wife gets home.
My passenger side front door lock stopped working on my 2004 LS430 a few months ago too. I live in Australia so the passenger side is on the left for me. It started off slow then got progressively worse until it wouldn't lock anymore with the remote and car wouldn't beep on locking.
Sadly in Australia, parts are not cheap and there's only 1 Lexus dealer in my entire state!! Lexus took the door apart and quoted me $1400 and stated I needed a whole new door closer assembly and actuator - the Australian part number was 69040-50220 which is only for RH drive vehicles and the part number was also for a series of parts. I was so shocked, I tried to seek help overseas from Lexus dealers and club vendors, but sadly either none could supply the part (due to LH vehicles) or even reply to my emails!! . Tried to get the part direct from an export company in Japan, but with postage and duty it was going to cost $900 just for the parts then I would have had to pay Lexus to install it anyway.
In the end I ordered it from the only dealer in my state and negotiated the total cost down to $1070 I couldn't risk taking it all apart and breaking something else, plus I didn't have the time and if something went wrong with a part I supplied myself then they wouldn't honor any warranty etc.
Good luck with yours, hopefully it will be cheaper to fix than mine was.

I think you are right with the actuator though.
If it works on the three others regularly, your actuator is dying. If you are familiar with small electric motors, the brushes are wearing on the armature. Sometimes they work, other times they don't. The motor will fail here shortly. Truly sorry to be the bearer of bad news. It's not a fun job at all.
