I checked the service entries on my Lexus cars a couple of times, and some of the wording used in them was hard to understand. I never researched things any further though.

if you register your car at lexusdrivers.com sometimes the reports are easier to understand from there.
the first event indicates a muffler replacement...unsure why that would be.
second event looks to me to be an exhaust manifold gasket. I am unsure whether these cars use a one-piece gasket or individuals per cylinder. If they are individual per cylinder, the gasket for cylinder 4 was replaced, which combined with the first results indicate there was probably an exhaust leak.
Mobile genius I got nothing. VVTi oil hose replacement is common.

thank you i did register with lexusdrivers.com
it doens't give anymore detail on them
i will go back to the dealer for the next car wash next week
and stop in services since it is the same dealer that car was sold and services at all the time
and ask them if they have a way of pulling records to see exactly what these are
thank you

I am also frustrated that Lexus doesn't allow the owner to click on any given repair and see the actual work performed and parts used for the repair. Shouldn't be hard to do, as Toyota allows the owner to do just that on their Owner Link counterpart. You could almost conclude that Lexus is purposely attempting to keep information from the owner, which doesn't make much sense.

Makes you wonder what services they charge for that they don't actually do. They also probably don't want you to have enough information to give to a good indy if you're not knowledgeable about cars to try and keep your business.

Bingo, IMO! Seems like the want to protect the Service Dept. business.
