I've tried both med and high. It's just the sensitivity, whatever worked for you before will be more likely to work on the first time, nothing else changes. I guess you can say it's faster if it works on the first time rather than the second.
I touch the emblem, so I don't know about distance. It's not a dramatic change.
One of these days I'll have to check what mine is set for....it works about 25% of the time. Otherwise I've gotten used to just using the button above the plate.
The sensor under the Lexus "Arrowhead" is specifically under the top half, do not fully cover the whole
logo with your hand, just the top. If you hold your hand "fingers up" your palm will be over the bottom...
Try holding your hand or elbow across the hatch lid, do not wave around, just hold it there until you hear
the chirp and then move away.
The manual mentions not to coat or cover the emblem with wax or other preparations. We sell a paint
protective and the crew was slathering it pretty liberally over the whole area...made the opening operation much
less consistent. Had to use a solvent to fully clear the dome, window cleaner didn't cut it, and that restored
the magic.
I noticed your body (the key fob) has to be close to the door too.
It wont open even though the emblem is your arms reach. I had to move close to the trunk.

Not that close. I was able to open the back door using the touch-free sensor while standing about a feet away from the vehicle (key fob inside my pocket). I have increased the sensor setting to high so I don't know if that has something to do with it.

I've coated my rear Lexus emblem and it works as usual. It's just a matter of getting use to how this sensor works (at least it was for me). Thanks for the elbow method tip as it works well since I have the remote in my pant pocket. I'll have my wife check if this works with the remote in her purse. Great tip!

Yep, I mostly use this method as I use the button on another vehicle we own as well that's situated in the same location.
