I often leave the car running when I leave the car with my phone since someone is waiting in the car. When I return, my iPhone does not pair to my car. When I go to my bluetooth screen, it shows my iPhone is paired, but CarPlay is always turned off. It randomly happened when I was using my iPhone 16 with iOS 18 or earlier. With iPhone Air, it happens every time when my phone is away from the connection distance. I have to manually toggle the CarPlay switch on the screen. Does anyone know of a fix for this?
Same with Android Auto. If there is a communications issue I think there is 1 or 2 automatic retries (because I have on occasion seen it come back on its own), but if no joy the system simply disables the feature. I believe this is baked into the core function of the system, no "fix".
It's strange because when I was using my iPhone 16 or a previous iPhone (forgot which model), it reconnected most of the time, which may take a min or 2 at times. With the iPhone Air, it never reconnect on it's own. Even toggling the CarPlay switch takes several tries.
Could depend on why the connection was broken and how each device responds. If the car believes the connection is broken, but the phone doesn't (yet), retries tend not to succeed, hence the backing off logic I suppose. But if both devices agree the connection has failed, they might be more inclined to rapidly reconnect. Differences could be due to minor software differences on devices or minor hardware glitches. WiFi and BT radios are probably the most glitchy hardware/software in consumer electronics.

I'm not sure which screen you are referring to.
Here is the quickest way that I have found to reconnect and / or select my iPhone if the system has selected my wife's iPhone if she got in the car first.
1. On Lexus menu to to Settings, Bluetooth & Devices
2. Select the iPhone that you want connected to CarPlay
3. Toggle the "Use For Apple CarPlay" toggle on.
It will start using the selected iPhone momentarily.

This is the screen I'm referring to. I only have 1 iPhone paired to my car since my other iPhone is for work that I do not pair. I find it strange that my prev iPhone 16 would auto pair when I return to my car. But iPhone 16 (or maybe its bc of iOS 26) doesn't. I have to manually toggle on the "Use for Apple CarPlay." When I return, it shows my phone is connected, but the CarPlay toggle off and doesn't turn on automatically. It only toggles on automatically if the phone is in the car when I start the car.
I’m having the same issue. Car will randomly switch to BT connection and I have to go to the screen above and select CarPlay. Doesn’t seem to be a pattern to it. Not sure if it’s an iOS 26 bug or something else?? Will probably delete the connections on the phone and the NX, restart the phone, restart the NX infotainment and re-pair. Just need to find a few minutes to try this.

I had to pretty much start over with the connection process, a new carplay code and all, and it took awhile to connect, but it did connect and seems to be working.

Yes, I’m having a similar problem. My 16 pro doesn’t reconnect to CarPlay, even though my phone is set to primary. It ALWAYS grabs my wife’s 17 pro. I deleted both devices, and re-added them. Same problem, always grabs my wife’s phone. Started with IOS 26 - but then wife’s 17 Pro obviously arrived at the same time.
Never have a problem with my old XR but to make it foolproof I always plug it into smart plug so it's a wired connection and charges - my wireless charger is pathetic on my '22. I've had it replaced 2 times and gave up. Plus on wireless I find Google maps speed display is always off, plugged in it's accurate.
