It turns over and the motor seems like it's about to start...then sorta sputters and doesn't start.
If I give it some gas, it'll shoot the RPM's up a little and the car will start.
Once the car is started, it runs fine.
Does it seem like the starter motor?
The beast has 120k on it and it's a 99
These cars rock like no other.
First thing I'd do is check the battery; strange behavior occurs when voltage is down. Fortunately, our cars start quite easily but this often serves to confuse the situation when strange things start to happen that seem to be electrically related. The battery in our RX300 lasted more than 5 years; the battery in my GS only lasted 2!! So you can't go by age.
Have dealer or your service guy do a load test first. This doesn't sound like starter.
I had the same experience back in late 2002 on my 99. To me, it sounds like something is clogged. I would sugget you used a fuel injection cleaner. Techron is good, but there is also a fuel injector cleaner from Restore that should do the trick. Put the injector cleaner in the night before you take the car out and make sure you have a full tank.

I was thinking it was possibly fuel related also...but my family has had tons of Lexus's
We've never had this issue before. And I've never had to use a fuel injection cleaner.
I thought problems like that were isolated to domestics
Sound like you got bad Idle Air Control Valve. This valve is located under the throttle body, if it get really bad you will have to hold the gas pedal for a few seconds(or almost a minute) and if you let off, the car will die. But once it's warm up your car idle fine. I've seen a lot of this problems and some deposit inside the valve(high mileage RX and ES).

Yup check your idle air control valve. I don't remember how many of these i have to replace.
