The PCV valve is unlikely to cause such issues. Spark plugs should be good, it is recommend to replace spark plugs at 100k miles.
Check throttle body and make sure it can open and close freely. Check both engine air filters. Clean both MAF sensors. Make sure all air intake hoses connected correctly. Reset the ECU and see it makes any difference.
If the issue persists, remove the spark plugs and check its condition.

If I understand you correctly, you are asking what signs of a faulty PCV valve? If it is what you are asking, I would say that consumption of oil is one of signs though it can be caused by other issues too. Before I replaced mine, I had a slight oil consumption. Also my PCV valve had a physical damage to it and was falling apart.
Why the air filter housing has an additional filter that cant be removed.i degreased both filters cause they were flithy.
well i cleaned my t.b today it wasn't bad but had a stubborn ring that I had to fight with..also cleaned both maf they wasn't bad either reassembled test drove much better..my next to do is the seafoam has anyone done this job yet...let me know.

thanks was looking for the pcv valve but couldn't find it..might be under the manifold. To much work kinda chilly for all that.

it’s a charcoal filter meant to reduce emissions. Something about fuel fumes. Something about California.
So one bank is rich and the other is lean? The question I have is do you have a check engine light?
If not then I’d do what everyone else is saying...clean this, clean that. Check for loose hoses and clamps. Check for an air leak getting into one side. Check your exhaust on that side. Check both sides...an exhaust leak can cause a lean condition and the computer will try to compensate. Same for an intake leak. Your O2 sensors could be faulty, but I’d expect a code from that...but then again maybe not.
What I would NOT do is waste any time or money cleaning the inside of the engine with sea foam or anything else. First off, yes the car is direct injected, BUT it is also port injected, so there shouldn’t be excessive carbon buildup on the valves. And if there was?? I wouldn’t expect it to effect only one side...however I guess the PCV valve side would be worse...but on this engine Lexus wisely placed the PCV in the center, so it won’t dominate a particular side. Other manufacturers will sometimes dump it right on top of a particular cylinder and years later that cylinder will be carboned up on that particular valve, causing misfires.
If your running rich on one bank and it truly is constantly doing that, I’d worry about a few specific things...air leak, O2 sensor or heaven forbid fuel injectors.
Update..did the sprayfoam today and man tremendous smoke exit threw the exhaust.smoke up the whole block..lol..also added injection cleaner.while driving to the southern state to air it out..trail of smoke lefted behind.felt bad for the people driving behind me..on the highway driving in sport mode enjoying the exhaust vents by Oliver..the car transformed to a rocket .happy I got my power restored .sprayfoam is the ******t..very happy with the results..thank you all for the advice..good job..get them exhaust vent from Oliver it will transform ur sound.
