The description + part number for the 10.1 DVD is "Toyota Lexus Navigation DVD Gen 2-3 v10.1 86271-33049 464210-1670". This is the one I'm using in my 2002 Gen 2/3.
After 10.1, the North American NAV DVDs are split between west and east. The relevant one for me is East, and the full description + part number for the 13.1 East DVD is "Toyota Lexus Navigation GPS 13.1 Gen 23 East U08 DVD 464210-2270".
I haven't tried 13.1 yet because I'm not sure how I feel about losing western data, but now that it's been a few years, I am pretty sure I won't do much, if any, driving in the western half of North America.
Someone correct me if I'm wrong, but I think the only difference between east and west is the POI list. Roads and mapping is still updated across both, but only POIs for your half of North America will show up.

Correct. POI's is the only thing lost.

Ordered and installed the 13.1 ver. dvd for my gen 2-3 '02 to replace the 6.1 which was in there. Happy to report that the "override" control is still available for the 13.1 so that came as a plus.
I contacted them regarding the same availability for gen 4-5 and they responded that the "override" is not available for them. Too bad but I am enjoying the new streets and roads on my screen now as opposed to listening to "on the way to your destination........"

The 2015 maps came out last week, but there was no update for Gen2-3, so the most recent is the 2013 "Toyota Lexus Navigation GPS 13.1 Gen 23 East U08 DVD 464210-2270" disc.
For safekeeping of the expensive original disc, I made a copy of it and use the copy in the car.
Note that not all DVD burners burn the same, nor do all blank media have the same quality/reflectivity characteristics. Using the same blank media (Verbatim DVD+R DL media code MKM-001-00) and same software (ImgBurn 2.5.7.0 - slowest speed, force booktype to DVD-ROM, otherwise default settings), the disc would fail to read 5 times out of 10 when I used my laptop's HP SU208GB burner.
When I used my desktop NEC ND-3500AG with the latest firmware, the disc reads perfectly fine 10 times out of 10.
Note that DVD burners can take up to 8 writes to the same blank discs using the exact same settings before the writing power strategies level out and disc quality becomes optimized. Therefore, if you use blanks that your burner has never seen before, try 8 times with the exact same settings before giving up on those blanks or your burner.
It is mandatory to burn DVD+R discs as DVD-ROM booktype for the disc to be readable at all in old readers such as the Gen2/3 nav unit. It is recommended to keep Optimum Power Calibration (OPC) disabled, which is the default anyway.

That being said, I don't think an incorrectly-burned backup disc would cause that problem. If the DVD doesn't work, the nav screen will say "Unable to read DVD". This can happen as soon as you turn the car on, or randomly during a drive. You have to power cycle the car to try to re-read the disc, but that's not a permanent solution. The permanent solution is to use higher quality blanks, making sure your burner has burned at least 8 of those exact same blanks recently, and setting the booktype to DVD-ROM.
If your screen shows a completely wrong location, I would stop the car while it shows the wrong position, enter the diagnostic screen, and look at the GPS signal status. Punch those GPS coordinates into a web browser on your cell phone and see what location it gives you. You can diagnose further from there after you validate the accuracy of your signal.
Also note how many satellites you are receiving from. You need to receive signal from at least 4 GPS satellites for a 3D measurement. With 3 satellites, you can only get a less accurate 2D measurement. With 2 satellites or less, do not expect your vehicle position to be accurate whatsoever.
If your GPS antenna is unplugged or intermittently defective, your car's positioning will rely more on autonomous navigation, meaning it is much less accurate and only uses 2 sensors to track the car: Vehicle Speed Sensor (for running distance) and Gyro Sensor (for direction based on angular velocity). If either of those sensors are less than perfect, your navigation will be even worse.
I once missed reconnecting a small cable in the trunk nav unit - this knocked out my GPS reception. I would double-check the cables back there. Worst case, copy your existing disc onto a better quality blank, or acquire another backup.

When I purchased my 2001 LS430 over 5 years ago, it had the original Nav Disc from 2000. I updated it then but I don't feel I need to update again as I drive locally and most streets are the same other than a handful of new communities. How much are they charging for the new disc? I'd rather save the money to either repaint my front and rear bumper covers or for some worn bushings when needed....
