I will be doing this project as well. 175k on the odometer and it's never been done. I can feel a little wiggle in the seat of my pants while at idle. I haven't raised the hood and stepped on the gas while stepping on the brake to visually confirm motor mounts are needed, but I don't mind sinking a couple hundred into the LS. She's treated me really well since purchasing in 2009!
It appears there are TWO mounts for the front. There is no left nor right part numbers meaing they are identical...part # 12361-50170 (about $170 each), and I believe ONE rear/tranny mount...part # 12371-50170 (about $105). GENUINE TOY/LEX obviously is the only way to go.
Can someone please confirm these part #'s for the 2004-2006 model years?

Start with the trans (aka rear) mount. It's the cheapest of the 3 and also the easiest to change. Quite often it will fix the issue by itself. If you replace it and still feel vibrations then go ahead with the 2 motor mounts...
Had a Toyota/Lexus specialist do mine mounts last month, asked him to take a photos because looking through the threads, I noticed there could be more photos especially looking up at the engine mount from underneath. I'm posting photos here for members who might be thinking about replacing theirs. I've an 04 non UL, car had ~120k miles at the time. I could feel some vibration from the engine at idle and in drive, figured if the mounts aren't cracked they are at least compressed.
Came across the Toy/Lex specialist while visiting family in Phoenix, bought OE mounts from Bell Lexus in Scottsdale during a sale at 30% off MSRP, and asked the mechanic to use cherry picker. The old mounts are noticeably compressed. With new mounts, the vibrations reduction is very noticeable, now it's not possible to tell if the engine is running at idle. In my case, replacing all 3 mounts took the mechanic and his apprentice a total of 3.5 hours.
Photos below to show the conditions of the old and new mounts, and also how the engine mounts look from underneath if someone attempts DIY. I specifically asked the mechanic not to jack up the engine from oil pan, and the process took a bit longer. Overall, very happy with the mechanic who recently left the dealership and started his own shop. Wish Philly/NYC tristate have more indie Toy/Lex mechanics like the west.
