What does this last picture setting do/mean? I know the first is recirculation, the last is straight air from the outside mostly. But this middle section idk what that does, any ideas??
Theres also the face option with a sort of screen in front and that I believe uses the cabin air filter but only for the head air flow.

It means it is on auto mode. Based on my understanding there's a smog sensor at the front of the car which assesses the air quality coming into the vehicle. If air quality isn't good (maybe because an old car was smoking in front of you) then it automatically switches it to recirculate.
Effectively just means it switches between the two settings for you.

In the last few weeks I've noticed mine was changing (after exiting and restarting the car) from the recirculation to the external (Auto switched off). I might give Auto a try when I'm out next.
Auto is nice but it doesn't always work perfectly for me. In tunnels for example, it rarely goes to recirculation by itself. However there is a way to adjust the smog sensitivity if you hold down the recirculation button while in auto mode - I've never personally messed with it though.
I also find the car will switch to external air if you turn the AC fans to off to allow for wind to pass through the air vents whilst driving along - so this may impact your AC settings when exiting the car?

I generally leave it on recirculation in both our cars, but I'd like it to switch between if possible. I just hate the damn diesel fumes particularly, or even people smoking!
With AC, I actually leave it on at all times. The only time I turn it off is if I open the windows, which is very rare unless it's summer.
I find it is way to slow to react too and manually switch it when down stream of diesel rigs.
That said, if you look at the HVAC module through Techstream, there are like 200 settings. I always intended to go back in and set the default blower speed for windshield defrost one step lower that high... Never have. But I'm sure there is a setting for smog sensitivity there too.

Wow, I don't know too much about Techstream (but I'm very computer savvy). I might have to look into it, is it fairly straightforward to use? A link would be great if you have one, otherwise I'll find it.

It's proprietary software and hardware for Toyota, Lexus, Scion. The real stuff from Toyota works, the pirate stuff less so. Do a search for Techstream, you'll find plenty of information here and elsewhere.
There is instruction in the manual to adjust sensitivity. If you have nav, it should be one of the settings on the screen. If you don't have nav, you can hold the button for a few seconds and adjust with the Temp up/down button. I adjusted the sensitivity a couple of times.
It does seem too slow to react. If I see smoke coming out of a car ahead, I manually change it to recirculate lol
