I have seen this posted a few times since I been a member ....but can anyone tell me what that noise is...? It only happens every so often when I start my car even after I have been driving it around...someone said could be bad gas but I switch up 3 different gas stations... Please someone give me a good answer on what to do about it...? Thanks!!

call your dealer and tell them you need your CAM sprockets replaced, I had the same issue you need to drop it off and leave it over night so they can re-create the sound. Might take awhile but you might get lucky. Its a known issue so the service advisor shouldnt BS you about it.
What about the cam gear though is making that noise? I don't think the gears are adjustable, are they? So what mechanically is failing on these gears? I'm definitely concerned, though I've got no noises right now.

It's not failing. There is a bit of a bit too much free play in the cam gears, so when the oil pressure begins to pressure up, it would make that ratting/knocking noise on startup intermittently. It would also do that once a while when the car is turned off as oil pressure drops.
I had this problem too but once I had the cam gears replaced under warranty, the issue went away. The new cam gears are built with tighter tolerances to address the free play do it prevents it from happening again. To me, the car sounds better on both startup/shut down after the work was done.
it will be under powertrain warranty even on CPO's but see the post from micel of what can happen if they mess it up.
One anomalous experience is not enough to condemn a repair. To gocubs08 - yes, this should be covered under the powertrain warranty, so take it in to get it fixed. I just had mine done a month ago and everything's perfect, no more sound. And if you're considering not getting this repair done because of the hassle or one person's unfortunate but anomalous experience, keep in mind the look on potential buyers' faces when they take a test drive in your car and hear that sound. Get it fixed now, while it's free.
I have an '06 IS250 with 56k miles on it, and the local dealer fixed the grinding noise two weeks ago. There's a TSIB for it and it should be free as long as you have power train warranty.
The dealer kept mine for a night to duplicate the noise (I doubt if they could replicate it as the noise was rare and intermittent), and after they replicated the noise it tool them couple of days to fix it.
