First thing would be to ensure that you have either the left or the right selector pushed, as if you're going to adjust the mirror. Doesn't matter which one (L or R), but one of them needs to be activated to ensure the mirrors tilt down in reverse.
If that is selected, then I'm not sure what to suggest.

Pretty much what JDR76 said. Beyond that, not sure what else to say other than you might need new parts?
My wife has 2007 RX 350 and you have to put in Left (L) position for auto tilt.
Same with my 2013 GS: if you leave the mirror switch in the 'neutral' position, the mirrors will not lower in reverse. You must leave it set to the left mirror.
In my 2016 GS, both the side mirrors tilt downwards when in reverse and the Left or Right has been selected. However, in both cases (L and R), my driver side mirror tilts more than my passenger side mirror. Is there a way to make the passenger side mirror tilt more?

Adjust the mirrors while you're in reverse. Both sides. They will default to however you adjusted them next time.
