I am trying to decide if I should take the car to the dealer for the 15K service. What do they typically do and how much does it cost? Will it impact my warranty if I don’t take it?
As far as I know, the only free or included services are the 5 and 10k mile services. After that, the cost is on the owner. Given that the recommended OCI is 10k miles, I would imagine that the dealers 15k service would only include a tire rotation and general inspection. No idea what the cost would be but certainly a call to your dealer would give you this information.
If you decide to not have this service performed at your dealer, I don't see where this would impact your warranty. Likewise with any future standard/scheduled maintenance, but I would definitely document any maintenance services (oil changes, etc.) NOT performed at your dealer for the duration of the warranty period, just in case.
Personally, I had the 5 and 10k services performed at the dealer but don't plan on using them again for general maintenance. I do my own maintenance (oil and filter, air filter, cabin filter, tire rotations etc.) and it is documented.
I have always done my scheduled maintenance ala carte meaning at 15,000 would tell them tire rotation. At 20,000 oil change tire rotation and I change the cabin and air filters myself. $45 for both five minutes to change. At 30,000 miles oil change tire rotation and brake fluid service. My guess would be Lexus will charge you $250 for the 15,000 mile service. I have never had a problem with any warranty issues.
The 10K, 20K, 30K etc. visits include the LOF w/ synthetic oil while the 5K,15K, 25K etc. visits are "light maintenance" and include the tire rotation and basic checks.
No obligation to see the dealership, the Lexus App even has a tab to let you enter maintenance details into the log that Dealership visits populate.
Dealership visits do provide recall and campaign updates and any software updates (some now OTA) that a quick lube or DIY efforts would not include.

I just had my 2024 450h+ get 15K service yesterday at my local Toyota dealer. $114. They changed the oil, rotated the tires, inspected. All good. I remember that after my 10K service at the Lexus dealer (last included service), the service rep quoted me about $220 or so for the 15K.
I'm very satisfied with the service performed at the Toyota dealership (Northshore Toyota in Covington, LA). I had every 5K service after the first two Lexus complimentary done at this Toyota dealership on my previous car, a 2019 450h. Never a problem.
I never went that far into the menu because I wasn't planning on making a dealer appointment!

Unfortunately, my COMPLETE 15K service (everything the dealer would do, plus a lot of EXTRA checks and verification) couldn't be recorded, perhaps since it was performed back in September. Kept coming up with a syntax error.
Our dealer experience:
On the 5K service, the lug nuts were not properly tightened when the tires were rotated.
On the 10K service, done at 8200 miles, they said they replaced the cabin filter but didn't.

The service advisor confirmed that a synthetic oil change is required every 10K miles, but I was a bit concerned and opted to get the oil changed at just over 5K miles, following the "free" one I had at around 10K miles a few months ago. I will be considering using services at Toyota dealership in the future.

$260 seems outrageous. Is the tire rotation more expensive than the oil change??
Is it possible they did a tire rotation AND balancing? I recently went in for a 20,000 mile service and they offered so many things that if I would have said yes to everything, it would have cost me $630. Luckily, I learned some great tips here. Even though i am basically inept around cars, I changed my own filters and added Techron to my gas tank and spent approximately $190 at the dealership
