IMO, CPO is a waste of money on anything none German. You are roughly paying a $4,000 dollar premium over a none CPO. Lexus is known for its reliability and rarely need expensive repairs. To each their own - If peace of mind is what you are looking for, then go for it. My only advice is that you drive the car and make sure it's solid and does not have issues associated with TSB issued for the 16 MY - AC gurgling, squeak from rear trailing arms, trans programming, and exhaust drone.
I always get the extended warranty on all my vehicles. Very cheap piece of mind. And the cost has always been cheaper than the repairs and regular service I needed. All 4 of Lexus vehicles have been back in for repairs after the initial warranty has expired.
I purchased a CPO 2016 FWD RX350 last summer. Its already been in the shop twice. The original warranty is just about to end. And the two years of extended will kick in. So far its had side mirror motor issue, Rear bumper cross traffic sensors came loose and fell off. The GPS location service was broken. They fixed it last year. But now its a service recall. My next oil change I will have them look at the Map button. When you press the menu button. Its works. But the map button is sticking. So you have to press it a couple times before the map display comes back.
My Last Lexus was a 2015 IS. It had ECU and air conditioning problems. I drove it for about a month. Then it was in the shop for about 45 of the next 60 days. They never figured it out. And It finally got fixed when the replaced everything.
I agree that CPO or extended warranty is a matter of peace of mind. I have it on my wife's Mercedes, as well as pre-paid maintenance, and on that car it makes a lot of sense. On a Lexus, maybe so maybe not. I'll probably get extended warranty on my 2019 after basic warranty expires in December 2023. As to additional money for a CPO Lexus, consensus is that it represents an additional $2,000 or so, not $4,000 as stated earlier. I would have only bought CPO if I had bought used. Again, peace of mind.

That's also my default position but I made an exception on Fri. when I bought my '17 CPO. I did so because finding a new or used RX F-Sport fully optioned in Atomic Silver/Status Gray is hard. Until this one, the last one I saw was 1.5 years ago. I regretted not buying that one. This time I got lucky. The '17 came in on a lease trade in earlier in the week. It'd just come out of service before I arrived at the dealership. Pics weren't even on the website. But I took a risk and drove 4 hours to check it out. I'd bought 2 cars from this dealership before, so I had a pretty good feeling.
One upside is that the '17 has 1 year of manufacturer's warranty left + 2y of CPO = 3 years of warranty. Not that I expect to use it . . . .
