I would not put it past some marketing departments:

Need a new design? No problem, just put a hole in it. Makes sense to me. What's your problem?

My Lexus RX front rotors do not have a hole as shown in your top pic. Look like your lower pic.

If the caliper bolts up then it is the correct (front) rotor.
The hole is just an error. New guy working.

I would call them and ask about it...smells fishy. I would expect the exact same replacement unless there is a clear advantage to any change (such as the water pump change after the 3rd gen that is a better design internally).

Bizarre.. I would measure the thickness and that would give you an idea if it's front or rear. Fronts are usually thicker by design.

Specs from RockAuto for reference:

-Front nominal thickness 28 mm
-Rear nominal thickness 18 mm

I have seen the extra hole in OEM replacement brake rotors several times. I think that during the manufacturing process the equipment is machining the hub sections of some brake rotors the same for front or rear rotors.

You can call the dealer, but the staff in the parts department don't know much about what is in the boxes they sell.

Multiple different vehicles might use the same rotor but require the use of a mounting bolt (BMW use them). The Raybestos rotors I just put on have the hole also. Not a big deal and does not affect the capability of the rotor.

Raybestos show the following Toyota/Lexus part #s.... OEM / Interchange Numbers: 435120E050, 4351248130, 4351278010, 43512F6010

I suspect these rotors might also be used in newer RX models that use lug bolts instead of lug nuts. If that’s the case, this hole is designed for a screw that holds the rotor in place until the assembly is complete.

I purchased the rotors from the dealership (P/N: 43512-F6010).
According to LexusPartNow.com, this part number supersedes 43512-0E050 and 43512-78010.
I’m still unsure about the purpose of the hole and whether I should replace the rotor because of it.
Actually MSRP for 43512-F6010 is almost $10 more than 43512-48130 on Lexus parts website.
