Tire & Wheel Road hazard Protection
I purchased the protection in a rush and didn't ask any question regarding it since I damaged 3 tires in one year on my previous car. But now I'm, not even sure if it is useful for me.
1) Lets say I get a nail on the tire. Do I just take it to the dealer and the will patch the hole, or fix it? If it is a small nail, can I just patch it and when I get another bigger nail, then go to dealer to have it replaced?
2) If the rims gets some scratches from parking, can I save the damage until they are very noticeable before going to dealer for repair? Or can I just go with one damage at a time?
3) The protection doesn't cover wear and tiers, but lets say when the tires are almost gone, I get a nail (you know, accident ), will they replace the tire?
4) If i'm too far from a dealer, Can I just replace the tire on the nearest tire repair shop and bill it to Lexus? if not, what do I do in this circumstances?

I can't really help you with 2 and 3 without seeing the contract language and what's covered.
As for 1, yes, you can just take it to the dealer. I don't have this coverage, but did recently get a nail in a tire of my new IS. I called my Lexus dealer and they said I could just swing by and they'd work it in. It ended up working out better for me to just take it to the local tire shop (Les Schwab Tires) about a half mile from my work, as they offered to fix it at no cost.
As for #4, if you are away from a dealer, I'd just call around and find the closest shop, as many will do this at no charge. I do not believe Lexus will be willing to cover the bill though if a shop does charge you. If you're away, you might be best to call your Lexus roadside service and have them handle it. That way Lexus will pay them directly. At least that's what I would do.

Just to add, if your tires are worn out, a good tire repair person will not fix a puncture.

I realized that I should talk to the dealer, but I wanted to have an idea from other drivers so I can compare them to the one I have. Thank you.

I like the COSTCO tire program more vs any dealer's.
And it's Free, as long as you purchase the tires from them.
And you can take the car to any Costco near you,
which is handy especially when you do road trips.
Check it out.
Some members sell their OEM tires and replace them with better/quieter ones like Michellin
I know it's a hassle but consider your history with nails. 3 times I a year is a lot
