Well corporate offered 50% so far. They claim the service manager said there appears to be outside damage to the case which caused it to crack. But how could that happen? I think it's vibration from a defect that caused the transmission to crack. Lexus told me to contact insurance company "to pay the other half". What next?

Since you mentioned it felt like something fell off the car when this failure happened and Lexus says there is external damage, maybe you hit some road debris like a piece of rebar that bounced up.
It's impressive they would offer any goodwill assistance in that case. Call your insurance company and see if they will send an adjuster to take a look at it.

Nearly 3 weeks later and here I am waiting. Corporate wants insurance to pay, but tell me they will offer 50% otherwise.
However, Lexus forwarded photos today of the transmission. Could anyone here tell me WTF happened?
Lexus says this was "outside caused damage" but I don't see how that was possible. My guess is that something caused a vibration in the transmission (the wind noise in post #1) which caused the case to crack and then break apart.

So it appears that this was not caused outside damage, as I had thought. I'm now humiliated that I had to ask my insurance to send an adjuster. The adjuster said it was the result of a long term defect which caused that. That same long term defect which I'd been complaining about that the dealer never looked at.
I'm asking corporate to increase their 50% offer of goodwill to something much higher. I had everything done at the dealer since I owned the car including tires, brakes, oil changes, etc. They should have found this years ago when the car was under both the powertrain and the extended warranty. I think Lexus knows about this problem is trying to cover it up so as to avoid a recall on these cars.
