Are you putting an increased octane (>87 USA) in? In summer I notice a mileage/perf difference on 04 RX330 with 89. Some stations only get a $0.10 premium over 87 and its an economical win/win (ie mpg cost goes down IME). Especially useful in hot conditions with A/C use.

Won't do a thing unless you have the rest of the vehicle tricked out (intake, exhaust, etc.) Of those, a direct flow muffler is the best bang for the buck and with California emissions where you are at, it is a viable option.
Otherwise everyone would be putting a chip or engine management in their vehicle. There's only so much you can squeeze out of a stock vehicle.

Yup, especially when the ecm is trying to compensate for every move you make.
