Default Is this a door lock actuator issue?

I've been doing some searching back and forth and I'm fairly certain my issue is related to a power door lock actuator

Symptoms are as follows:

-drivers side will not lock with the remote or button on door (intermittently)
-door can only be locked manually with the key from the exterior
-door lock itself will stick at times and I am not able to open door, or lock from the exterior with the key

I would assume that if the motor in the actuator is indeed dying this would explain why the lock itself sticks sometimes and is not able to move. It's almost as if the motor is seized completely at times.

I have already found the DIY for replacing the motor for around $5, but before I tear it all apart I want to be certain that the door lock actuator is the faulty part.

It sure sounds to me like the actuator.
