For the basic 48 month/50k mile warranty does the clock start ticking from the date the vehicle was manufactured or the day it leaves the dealership when the first owner takes possession?
You misunderstood. "Service" can have multiple meanings. The day the first owner bought and drove off in the car is considered the "first day of service".

I can't say exactly, but it seems the owner picked up the car in August 2019, which would be the first day of the warranty.

It would make sense the warranty starts the day the owner took possession. The reason why I think the guy bought it in 2020 is because based on the other records it looks like he drove the vehicle about 1000 miles a month so having less than 3k on it on September of 2020 would make me think he definitely purchased it in 2020. The other records with only 5 or 6 miles just seem to be Lexus performing initial services on the vehicle while it was in their possession.
