Default Brake actuator problems

Which model years of the LS have the most brake actuator problems? What is the first model year where these problems seem to have been corrected?

2007 to 2012 from what I understand. That's what Lexus issued the extended warranty bulletin for.

Can someone confirm this is the OEM brake filler cap that some folks replace to ensure a tight seal on the brake system?

Thanks. I was not aware of the extended coverage on the brake actuator. My LS only has 18000 miles on it, no problems to date, but I was aware of actuator problems from the forum.
