Thought I'd pass this along. I was unable to delete any of my old Iphones from bluetooth so I was unable to add my new phone, since I was only able to register 4.
Lexus service told me to press and hold power/volume button for three seconds. Although I saw nothing change on the screen, somehow this solved the problem. My phone simply paired and I guess the others were deleted. I never checked but I believe holding the power button sets the system back to default.
I have no problem pairing, but it is not reliable. It's like I can only have 1 call--after that, there is miscommunication with my phone. This includes a iPhone 6, Xs, S8, S4. All of these phones had no issues with any other car. I know it sounds weird, but if I am expecting an important call, I make sure no others come in before it. Or, I shut it all down and restart.
My iPhone 6 behaves weirdly with the car bluetooth. It will receive an incoming call, but when you try to dial out it misses the ringing part and only activates the BT speaker once the other party has answered and you miss the first 2-3 seconds of speech which is awkward.

I have the same exact issue...it worked flawlessly for months and then started this same behavior after a software update.
