New belt assembly.
I've seen it before on other aged Toyota's...it wears out with use. By then, the belt is fraying too. Safety first, replace it.
There was a recall/safety check on the tensioners. Both mine ('97) were replaced in the car's early history like sub 10K miles.

Seat Belts/Lexus Customer Service
Yup. Call Lexus Customer Service about the seat belts. They will give you instructions on what you can do to have the seat belts replaced. Use Search-- this has been discussed previously in the forum.

I Can't find a replacement for the driver side. Would a passenger side seat belt assembly work?

You can have your belt rebuilt by services online if you can't find a used one, or maybe you can try cleaning it first
An example of a rebuild service (may not be applicable if you don't need the pretensioner replaced. You may need the retractor repaired)

No. The passenger side assembly is different.
You can have your belt rebuilt by services online if you can't find a used one, or maybe you can try cleaning it first

You're probably having trouble finding a driver side belt assembly because most of them are already FUBAR because they're the most used.

The Safety Restore link should be for a general repair service. I sent that one because I don't know what's bad with your unit
I should note however, that link may not be the solution for you
You may just need the retractor repaired / rebuilt. Probably worth searching online
I'll update my post to reflect this more accurately
