Driver's side. It is the only thing on that V bank with a hose attached to it. You don't have to remove anything. Of this is off of an SC4, but I don't believe it's any different on LS4.
Today I changed the PCV on my `99 LS with 107K miles. Most likely it was the original one(i got the car with 38K miles). I was surprised that the old valve wasn`t much different in condition from the new part, no build up like on LexLS tutorial picture. The grommet was in a lot worse shape though, old rubber got brittle and i had to be extra careful not to drop anything inside the whole - the grommet broke into pieces as I was taking it out and I had to use both little long nose pliers and a screwdriver in the other hand to hold the breaking parts from dropping inside. The area around the valve was a little dirty - perhaps due to the grommet not being tight.
