P0023 after changing spark plugs

You can probably follow the DIY for the IS250 spark plug change since they have to remove the IM during plug installation. Should be very similar.

My mechanic came through and swapped the bank2 exhaust OCV and cleared the code.
While he was swapping the OCV the connection on the valve broke off, exposing the pins.
I'm guessing i stressed the valve too much when removing the first coil pack.
It's been a couple of days and no issues so far.
Thanks for the help I appreciate it a lot!

Yeah, this isn't the first time this exact issue had been posted about on here. Usually the member just completely pulls the wires out of the sensor itself.

Understand this to be an older post however, encountered the similar issue after changing plugs (doesn't downshift / kick into gear hard when gas is applied). Installed NEW DENSO FK20HBR11 / 3473 - OEM Part 90919-01249 plugs. Did not remove the intake manifold, no error codes were received afterwards or currently present, and no erratic idle . Have double checked the coil connectors and connectors disconnected to move harness for clearance to spark plugs. I applied thin layer of Permatex Spark Plug Boot Grease to inside of coil boot tube that installs over spark plug porcelain end. Also added dielectric grease to connectors prior to re-connecting. Confirmed the coils / connectors are in correct order (Grey / Black / Grey) on both sides. I've replaced the plugs before on this engine and didn't have this issue afterwards. Is there something that is commonly overlooked and missed during spark plug removal / install?

Encounter the same but have no error codes?
Maybe I missed it, but his issue was an error code and unable to start on first try.

Yes, posting is P0023 after changing spark plugs. Perhaps should have stated similar issue (updated). But understand post was perhaps misleading not having the P0023 code.
Thought there may have been something that's commonly overlooked in addition to unconnected connector, broken / defective connector wire that my have caused the issue. Am grateful no CEL encountered. May jut remove the ETCE & EFI1 fuses to reset, though haven't had to do so after previous plug changes.
