Our New Lexus RX 400h
I performed the ISC reset myself on the 400h and it worked that day. The next day back to 19 mpg. WTF. So I had my wife take it to the dealer for the ISC reset. They performed the ISC reset, but it would not "take" because the Auxilary Battery (front) is going bad and would not hold a charge.
They installed a new battery ($99 for the battery & $72 to install; what was I to do, my wife and baby was waiting) and perfomed the ISC reset. I figured the battery needed to be replaced, it's the OEM battery and it's 5 years old. On her way home she received 27.5 mpg (40% city / 60% hwy).
Therefore, if you vehicle is getting less than 23 mpg and the ISC reset does not seem to be working....check the Aux battery (espcially if it's older than 3-4 years)

Took battery to be tested and sure enough, it's rated for 450CCA, but only had 170 or something. Now, we don't need to worry about CCA for our cars, since there is no starter motor, however, the low CCA showed that one or two cells in the battery were kaput, which seemed to be affecting the ISC (maybe losing the programming overnight due to the weak battery?)
Anyway, put in a new battery myself and it's been good ever since (and I can leave the door/stereo on for longer than five minutes).
