Great news, Lexus has come up with an updated part for the display assembly. It is a pricy part, but it is a direct fit and doesn't require teardown of the screen:

Below is the link to the part. It cost me $110 USD. When you click on the link it might say it's a risky website. I'm not sure why it does that but it's a link to the AliExpress website. If it does not take you directly to the part search for this item within the website:Originally Posted by Newguy79
Would you mind posting a link to the seller so I can make my purchase?

Has anyone successfully upgraded the system in the Lexus 250H to Apple Car Play? If so, where did you get the parts?

Not really unless you get the $99 option linked at the end or something similar.
See these threads. It is very expensive, you have to gut the dash and install new stuff that does not have good instructions and which may be lacking all the connectors. Youu are running an experiment on your car that may fail and leave it worse than before. This is a very old vehicle and it is just not possible to easily and inexpensively upgrade the infotainment system to carplay. You have to live with what you have, or trade it in, or get a really good dash phone mount or dash mount bluetooth connected screen thing.

Or get something like these and mount it on your dash and hope it works with our old bluetooth.

I attempted to install the Toshiba screen identified above, but the screen wiring in my 2011 HS was different. On this screen the smaller connector is on the side. In my vehicle, it is on top and closer to the center. It appeared that the side mounted wire was too far away to connect. It also appeared almost impossible to do the final screen disassembly to reach those wire connectors and the service manual PDF listed above was not much help. Then there were the 2 micro size silver color screws I stripped and had to drill out to remove the metal frame around the screen!
I put it all back together and luckily it all still works. Photos were a great help with all the tiny screws and the alignment of the plastic pieces etc.
It was stuck into the dash and took some effort to yank it out once it was unbolted. There are only 2 bolts in front and the rear has tabs that slide into holes. Then there are 3 wire connectors at the back that have to be separated to remove it. Once removed there are more wire connectors to separate and many more tiny screws and plastic parts to pull apart.
Tracking the tiny black machine screws vs the tiny black screws for the plastic was half guess work on reassembly.
