Because of the longer travel needed with the accelerator pedal, that alone should lead to more gentle acceleration and better fuel economy in ECO mode. Also, I believe that there is a change in the shift points. In ECO mode, the transmission will shift to a higher gear at lower RPMs than in the other modes. I suspect that any small differences in how the AC functions in ECO mode (with the ES350) would result in only marginal improvements in fuel economy, but the gentler acceleration and the change in transmission shift points are likely to result in more significant fuel savings.

in city driving, I'm lucky to get 20-21, with all the stop and go here it's actually more in the teens. On the highway (with the AC on) I was getting over 30. Most of my miles have been highway miles >80% so it's always jarring when I run errands in town and the mpg drops so sharply. TBF it's been a hot summer and it's a heavy car.

I have found with my daily commute to work, the ECO modes in both the ES and GS I owned, made a difference in my fuel efficiency. When I first got both models I didnt want to do much traveling with my paper tags so I commuted back and forth to work almost strictly. In normal mode it took me about a week to go through a full tank, In eco mode 2 weeks. I agree with lesz theory with the throttle/throttle/AC.
