On our car ...
Decoupler pulley froze at 150K and fried the alternator. Car was well over warranty so didn't even try to contact Toyota - manual does not cover this item. Gates says decoupler should be replaced every 100K.

VVT-i oil line burst at 138K. Also past warranty when it burst and I fixed it. Toyota later issued a voluntary recall and offered to pay expenses to those that had already fixed the problem...gotta find my receipts...

Issue with door windows falling off their tracks. Never happened to us and don't know if Toyota has been contacted by those affected.

Transmission flare. TSB out there. Also not an issue with our car.

Timing cover leak; some worse than others. Ours seeps when using 5/30; have been using 10/30 oil since about 120K which significantly reduced seepage. No recall issued but there may be a TSB. Our engine never required oil make-up.

Engine torque strut, aka dog bone strut. I just ordered the 4th replacement. Because of the rubber covers, most don't notice the tears in the rubber bushing, but I bet most of yours will be cracked.
