Stumped on clutch/trans issue on 2jz/r154
Hello,
First time posting here. This site has been incredibly helpful in my journey of building this 93 sc300 with a large, single turbo 2jz and r154 trans.
But I am stumped. I've installed an ACT 6 puck clutch along with a DM Evolution flywheel. After finally installing the driveline (MK3 supra) this morning, I took the car out for the first time.
I was surprised how little clutch travel there was. Bled the clutch line again and took it out. Clutch felt the same. The car is making about 500whp and the clutch is advertised to handle up to 650ft/lbs of torque.
About 5 minutes into my test ride I noticed the pedal was getting stuck depressed. I pulled it back up with my foot and the next time I depressed the pedal, the hub pulled through the clutch pressure plate.
After lifting the car up and doing a quick inspection, looks like the little lip holding the hub behind the pressure plate broke off.
Now I'm puzzled why it pulled through. From what I read on various forums is that most likely I installed the rings improperly. Unlikely, but I am human. Maybe I made a mistake. Any thoughts?
Just for the record, I ordered all new parts from DM. All parts were new.

Update:
Removed the trans/clutch. Found that the pressed on lip of said clutch release bearing was pulled off.
These are the diagrams I've been using to install the clutch.
If you look at the sketched diagram, the bearing provided by ACT (PART B) is 2 pieces. The lip is pressed on. In the short time that I went driving, when hub assembly was pulled in, the lip on PART B came off and the rest of it pulled through the clutch cover PART E.
I don't see how I could have installed this a different way.

Really need some help here.
I installed the hub again EXACTLY the way it was before. What did I do wrong?
Could this be a different issue? Good install on the hub and bearing, but something wrong with the clutch pedal/line/master cylinder/fork?
Another thing worth mentioning that when this hub/bearing is assembled and installed, there is quite a bit of play. the hub/bearing moves around pretty easily in the clutch cover as if the hole in the clutch cover is too large for the bearing.
This "lip" is pressed on to the clutch release bearing. Somehow it was pulled hard enough to come apart.
