That's old transmission software. It needs to be updated. The car disengages (goes to neutral) at a full stop, even with the car in 'D' - this was a first-generation transmission algorithm designed to save fuel. It became so annoying that a new version has been offered that makes the car stay in first gear and ready to go when at a full stop in "D". Go ask for the update, it's a TSB.

Now, if it revs up between gears (car already moving) that is also a transmission software thing, but it cures itself overtime, as it learns your driving habits.
