1996 LS: Plug the Windshield Sensor hole with rubber stopper
My LS' windshield washer light was on and the washer reservoir leaked fluid. I removed the splash guard and inspected the reservoir. The fluid leaked from the middle of the fluid level indicator sensor. The plastic shell of the sensor was broken. The sensor itself sits on a rubber grommet, which was mounted on the bottom of the reservoir.

The grommet was placed back to the hole at reservoir. The grommet was plugged with this rubber stopper from Lowes.
The Hillman Group 1-Pack Neoprene Hole Plug, Rubber Stoppers 13/16 x 5/8 x 1 , Item #: 139549 | Model #: 881323

The rubber stopper was placed at the grommet from inside of the reservoir, so that its bigger end was inside the reservoir, and the smaller end faces down and out. Silicon calk was applied to the outside of the rubber stopper and grommet.

I had the same problem. My sensor was fine, but the grommet had disintegrated. I wish I had looked around for a new grommet and just plugged the hole. The sensor/grommet combo is $50.
