Default Transmission Surges

I have a 2007 IS250 AWD. For the past year or so I have had an occasional problem with the transmission surging and sticking at about 4000 rpm's even though I am not accelerating. I have reported it to the dealer a couple of times and they can't replicate it and say they have not heard of this problem. The told me to use premium gas and that should resolve it. I have done that for the past year and it's only getting worse. It is happening every time I drive now. At low speeds and at higher speeds. When it does it I press firmly on the gas and release and eventually the RPM's will go back down to normal. Any ideas?
