2017 GX460 with app. 95000 miles. I decided to change my EGR valve after GXBOB video. There were no codes, Fuel mileage was app the same as always. I just wanted to show the amount of dirt and debris on the old valve. . The tubes were gummy also. Most of my mileage is on the highway, as I don't think that was a factor. Glad I did it but shows how important maintenance is if you want it to last. 2017 GX460 EGR with app 95K miles
That EGR valve looks fine. What matters is the mating surface and it seems to be clean. You could leave it alone or clean it and no need to replace it.

EGR has been on my mind too. I've got a whole 100K on top of you (195K on my 2017!), and I'm still fine with the stock EGR AFAIK. 95% of my driving is Highway. Not sure if Lexus would have cleaned it on any of the CPO/VSA warranties for Cylinder Head Gasket replacement or Coolant Valley Plate re-seal - but I doubt it.
Speaking of proactive maintenance - I will say transmission fluid is one to pay attention to as my GX Transmission fell apart around 144K Miles w/o any fluid changes (CPO VSA warranty saved the day) - where the transmission in my 2013 4Runner went past 235k with a full 13qt flush around 90K or so (same driving style). The unlimited mileage CPO VSA saved my *** with well over $10,000 in repairs over the years - now I'm trying to get the GX to 250K Mi with a similar DIY maintenance schedule as I had with my 4Runner (including flushing the tranny as it has over 50K on the reman tranny and a lead foot). Also keeping the coolant pristine after all of the CPO work (also got a new radiator) - hoping the other cylinder head gasket holds up. Maybe 200k is time to look at the EGR valve? Never did one on my 4Runner that never needed any warranty repairs when it was totaled at 235k. I'll be big on preventative maintenance items, but likely not proactively swap parts unless absolutely required. Shocks are currently a gray area (not "broken" but certainly worn).
If you haven't done coolant yet - get on it Head Gaskets are not a cheap fix on these, and I'd "Short" the 100K maintenance recommendation by at least 25%. EGR replacement is a distant 2nd at only 100K IMO...
