Default Oil conditioner

In my service manual it says at every 5k miles to add oil conditioner, What is an oil conditioner?

Are you sure it doesn't say something like "check the condition of the oil"? Otherwise, can you post the exact section and page of your manual?

Nothing like that in my 2018 RX 450h service guide.

In over 60+ years driving I never came across something like that. If my car needs oil conditioner, I'd just change oil.

Nothing like that in my 2018 RX manual. I might change my oil in 5000 miles. I want to treat the first change like break in oil.

An additive recommended after an oil shampoo.

How about STP? I remember adding it to the Castrol 20W-50 in my 1965 Sunbeam Alpine to thicken the oil.

You're probably looking at the "dealer" Lexus Maintenance guide rather than the factory Lexus Warranty and services guide. Noticed you are from Arizona and may have also gotten a dealer and factory guide as I did. My Arizona dealer guide for my 2016 450h says to get oil conditioner and fuel additive every 5000 miles along with the services which are listed in the factory guide. Also noticed the dealer guide shows for every 30,000 miles to get throttle body service, fuel injector service, internal engine decarbon in addition to the factory guide recommendations such as brake fluid exchange. Also, the dealer guide says to replace engine coolant at 60,000 miles whereas the factory guide recommends at 150,000 miles. Spark plug change, both guides were the same at 120,000 miles. I follow the factory guide, as most probably would, as the dealer guide, which is dealer specific seems excessive. Especially since the factory guide also includes multi point inspections.

"Dealer Guide" hahaha, the only thing guiding them is money, they want yours. The trickery that dealers get up to is obscene, many people would fall for this, Lexus should not let this happen.

Obviously the manufacturers maintenance schedule is fine, they don't want 100,000's vehicles dying in a few years time.
