I am considering prepaying 5 years maintenance, anyone out there can give me an idea how much I should pay?
Currently, the car is 4200 miles, and with one free maintenance left at the 10k service.
Also, the extended warranty, can I purchase out of state and still use it in any Lexus service location?

You can also shop around as I hear some dealers are much cheaper than others. You should be able to use the service at any Lexus dealer.
Pre-paid maintenance plans are almost pure profit for the dealership. If you want to do yourself a favor, not only skip the pre-paid maintenance plan, but, also, find either a Toyota dealer or a good independent mechanic to do your routine maintenance, and go back to the Lexus dealer only if you need warranty work done
For the last 15 years, I've had all of my maintenance work on my Lexus vehicles done by the local Toyota dealer. For the first 6 years of maintenance, which would take you up through the end of the 5 year pre-paid plan that you are considering, it will cost me under $550 to get all of the recommended maintenance done at the Toyota dealer. I'll bet that the pre-paid plan that you are being offered comes with a price tag that is several times that much.
Similarly, extended warranties are huge profit generators for the dealers. The mark-up on them is typically several hundred percent. If you put the money that you would have spent for an extended warranty into an "emergency repair" savings account, you are virtually guaranteed to be at least a couple of thousand dollars ahead by the time the extended warranty expires.
If a dealer can sell a customer an extended warranty and a pre-paid maintenance plan, that dealer can sell a car for a huge discount off from MSRP and still make the deal one of its most profitable ones.
Pre-paid maintenance package is worth it if you are getting a Mercedes. They actually replace the cabin,air filters and they cost A LOT.
My cousin's GLE 350D dealer wanted $900 for one service. Service B? I bought the parts needed for $3-400 I think. The diesel oil is freaking expensive and it took 3 cabin air filters.
When purchasing new, they offer the whole 3 year package for $1,200 CDN or something.
Lexus offered 3 year package for $1,400US. I asked for the service chart. Read through.. basically 3 oil changes and others were all inspections. Still have to pay for the cabin/air filter top of that.
I laughed and said no thanks.
Five year maintenance plan for the average driver would take it up to 60,000 miles. In that time you would need two brake fluid changes. Just yesterday got that done at a local Toyota dealership and cost $133. You would need six oil changes. Had that done yesterday and they charged $20 for the labor. The filter bought on line for $5 and the Mobil 1 bought at Walmart for $23 for the five quart jug and $6 for six one quart containers. Air and cabin filter would need replacing twice in that time. Bought these on line for $18 a piece. I am a left-handed mechanic and it takes five minutes to change both. With the cabin air filter behind the glove box make sure you have the A/C on recirculating. Total for five years for these would be $662. If you go in and ask for the 10,000, 20,000 30,000 maintenance you will be paying big bucks for a lot more items you do not need checked and inspected.
I don't work for any car maker but I know that prepaid maintenance plans are highway robbery. I'll have Lexus service my vehicle as needed.
If you get the maint agreement at the right price it does cost less than if you paid per service so they can save money. How do you figure that is highway robbery for someone who want Lexus to do all of their service?
