Best guess is rattling water pump a common problem on these engines.

I agree that it sounds like a failing water pump and this is not uncommon on these engines. It failed on my Sienna (same engine) at only 37,000 miles.
How many miles are on your RX? What model year?
The repair cost at an independent shop was around $1,100 with new coolant, and a new serpentine belt. Depending on mileage they may want to replace the belt tensioner and idler pulleys etc.
The cost will vary depending on where you live and where you take it to be repaired. It is about a 5 hour job.

100% water pump failure. You might be able to drive it a little longer short distances but dont try to go anywhere far with it. check and see how bad its leaking, and see my thread for all the fun i'm having doing this on my car.

I suspect water pumps fail on this car due to the coolant not being replaced enough but at the end of the day it's a known wear item on these engines.

Thanks for the follow up. Always nice to see/hear the video and then get the correct diagnosis. Helpful to other members who may have same sound/issue.

Curious what the cost for the repair will be.

Good luck with the repair.

They are not removing the engine. Really should be a 2-3 hours job for labor.
