buying from a lexus dealer
Part of the CPO process involves doing what's known as a load test on the battery using a battery tester, I seem to recall that its called a GR8. I know that the techs attach a copy to the CPO checklist.
If memory serves, the battery has to be around 70% of capacity to pass CPO standards; most of the time when a battery "fails" the car starts up just fine.
Like most anything consumable on a CPO the standards for replacement are much higher than what you or I would do on our own cars.
Edited to add: If you want actual specifics I can find out tomorrow.

For what its worth, I bought a 2013 RX-350 CPO in February with 22K miles, and they replaced the vehicle battery during the CPO inspection. No idea why. While looking on line before the purchase, I saw several other RX's that had their battery replaced and thought that was odd.
They will do a load test and only if it fails will they replace the battery.
A lot depends on geographic location and temperature. Heat will accelerate battery degradation.
I traded in a 2012 CT200h and a 2014 Ford F-150 Lariat last year to the local Florida Lexus dealer. Neither car had any indication when I owned them that there was a battery issue yet the dealer replaced the 12 volt battery on both before putting it on their used car lot.

A battery costs $120 tops. But perception is worth a lot more. When buying a CPO, you feel like buying almost brand new. Return on Investment. You know that pissed customer will never be happy again, when one morning the engine would not crank.
