Rear Wiper
Replace the entire blade, not just the insert. Possibly the blade is worn/damaged? No idea what it costs at the dealership, but there are aftermarket available. Do a search here or lexusownersclub - there was a recent DIY posted regarding this. Just did it myself recently.
i am happy with the factory coverage, i am simply looking to replace OEM.

So you want to just replace the old blade with a new one?
You just buy the correct length. Pull the cover off of the end. Where it connects to the motor. The whole assembly pops up. Pull out the old one. Install the new one.

kiwi, there is a TSB on this. I'm not sure what the fix is since I never did it. You said you're out of warranty so that doesn't help
But that annoying little area it doesn't clear can be annoying. I would pick up the wiper and drop it on to the glass. Sometimes it worked, sometimes it didn't. I noticed it happened more the first few swipes and got better. Maybe a newer wiper arm would help

I had this problem when we had our '04. Lexus replaced the whole arm & blade at NO CHARGE. It was still under warranty then. They said Lexus was having an issue with the spring tension if I remember correctly.
