No two machines are identical .. even the ones that roll off the assembly line .. brand spanking new.
CPO is essentially vehicle with extended warranty and hence the difference in price.
So your question really is should you buy extended warranty. Answer to that depends upon the price , your risk taking ability and matter of chance.
Lexus are for the most reliable, but your perticular vehicle may develop problems (hopefully not). Insurance companies make profit from selling the warranties.
My advice would be to pass on a vehicle with no-maintainence records. No/poor maintainance for sure will result in more problems.

...the CPO warranty MAY be worth it. CPO adds $1,000 to the cost of the car. You need to devide if that $1,000 is worth the warranty for the above 30,000 miles you may drive. If you can keep it under 30,000 miles there is exactly no reason to get the CPO warranty.
We were faced with this exact same question on a 2004 earlier this year and had the dealer knock the $1,000 of an already CPO car and take the warranty off. It was worthless to us.
