No Fourth!!!!!

No fourth gear! (o/d)
Sometimes it'll go into 4th, sometimes it won't.
My tranny just down-shift's and up-shifts randomly whenever it wants.
What can be causing this????
I can't drive like that anymore, my engine is starting to knock and I don't have anything else to drive.
