Software issues are out of control
A quarter of the time, CarPlay just refuses to work and constantly says CarPlay connection error even if I go into devices and try multiple times to connect to the phone.
The other quarter of the time, my profile straight up doesn't even load with my linked key. I've even seen instances where the speedometer screen says "Welcome [name]" and yet the system still doesn't load my profile when I start the car.
Is there something I'm doing wrong?

We tend to have connection issues when we switch from my wife (primary driver) to me. Two things seem to help. Like you, we have issues connecting maybe 25% of the time.
1. Waiting 10-20 seconds in park after starting the car before putting it into drive.
2. If we start driving and it still hasn’t connected, we reboot the system by pressing and holding the power/volume **** for 3-5 seconds. The reboot only takes a few seconds and that generally clears things up. We do this even while driving.
A few times when she’s alone, my wife has had to reboot and it always seems to work.
Hopefully a software update fixes this at some point.
Hope an update is coming soon, as it's almost 6 months of bug fixes should be coming....
If I forget and I put the phone on the charger before the profile loads then it doesn’t load.

I found it helps to leave the car locked and have the driver unlock the car when they're ready to leave.
Also, per Lexus' Tech Tip on the 'Profile not loading' you should leave/turn the ACC Customize function to on. When on, it does 2 things, your music/infotainment turn off when you turn off the car (vs. opening your door), something I like, and it also has the car looking for your phone while it is off. You can see this by looking at your Bluetooth settings and you'll see several ne BT devices with similar addresses. This is supposed to identify a profile by their phone prior to the person getting into the car. This probably doesn't help if both of you get into the car at the same time, but may help if only one is doing so. This will increase the drain on your 12V battery, which is why Lexus turns it off during shipment and dealers are supposed to turn it on during PDS.
The whole profile thing is a mess.... Anyone getting the PIN pop-up message while driving? We get it occasionally on the highway. If you're in a low network connection area and your profile times out (seems like they renew the profile validation) then you'll get the profile PIN pop-up. Makes driving, NAVng, or pretty much anything difficult. Lexus' answer is park in an area with decent cell coverage (if I had that where I lived I wouldn't get the problem) and enter your PIN.

'Customize ACC Function The pre-startup function detects when a user enters the vehicle and energizes the audio head unit so that the multimedia system can quickly be used. This function is not enabled at the factory to reduce parasitic current draw in transit and storage. To enable this function, it is necessary to set the ACC customization function in the navigation display to OFF.'
It also, when set to 'Off' leaves the head unit on after you shut the car off until you open the door.
