Resume not working on adaptive cruise control
Whether I tap the brake or the cancel button, I cannot recover my speed with the resume button. I had adaptive cruise on a previous Lexus and it worked correctly. Am I missing a setting? I have searched.
It should be as simple as that. I assume you have been able to use the same button to increase speed setting while cruise control is active (to verify that the button itself is working properly). You didn't mention your speed or situation when trying to use Resume. The only other thing I can think of (which I personally haven't tested) is that the cruise control is not settable under ~20mph. The manual is not clear whether that applies to "resume" or not though. The only mention of using "resume" outside of typical highway situations is to re-engage the cruise control after a controlled stop (ie the cruise control itself has brought the vehicle to a stop) and this only applies with the vehicle being held stopped.
Some things for you to try anyway...

I can only add that just be sure the "resume" is shown being selected in the HUD when operating the button. Since I don't have this, all my steering wheel buttons are always mapped to a specific purpose, but if I recall on the HUD setup the buttons can change their purpose depending on situation and personal settings. Sorry can't help you further, maybe someone more familiar with HUD buttons.

Also, make sure you’re on the correct button menu. The left and right sides of the steering each have one additional menu that you can access by pressing the menu button on either side of the wheel. Default first menu for the left side is audio and phone and default first menu for right is cruise control, and neither of the defaults can be changed.
Again, not doubting you know all this. Just making sure we’ve covered all the bases before delving further.
Is this issue you’re experiencing intermittent or continuous? Have you noticed any issues with other button commands not working? Are you or a passenger able to recreate the issue and take a video showing what’s happening?

Yes, as Droid13 said, ensure that the “resume” option is being shown before you press it. That’s why I asked if you have HUD. As I’m sure you’re aware, you kinda have to ‘wake up’ the buttons first (either by rubbing your finger over it or quickly pressing any button)—without doing this, pressing the “resume” button wouldn’t do anything.

Glad to help! If you’re able to recreate a scenario where it doesn’t work, let me know what you do and I’ll try to recreate it. Hopefully we can get this solved.
Another point to note here is that if you don't disengage cruise and allow the car to come to a complete stop in a stop and go traffic scenario, it will show 'waiting' on both HUD and center display. after a short time it gives up waiting and asks to either push down on throttle peddle or push resume on your steering wheel toggle once car in front starts to move. I've been just barely touching the throttle and it reactivates no problem. However this is only if you actually come to a complete stop without disengaging by using the brakes. If you've touched the brakes you can follow the resume methods noted by others.
It may display resume as a way of saying 'do you want to resume?' and wants you to hit the resume toggle to confirm. But if there was something that briefly changed it may decide not to resume. A bird flew in front of you, the car in front is now too close to resume etc. Keep in mind Toyota is THE MOST CAUTIOUS automaker in the world for safety, and I'm sure cautious of legal actions (floor mat fiasco is to blame partially). Their safety logic can be very unnerving to say the least. Yet they couldn't design a driver's side mirror that doesn't shake even after the Rav4 design flaw!

Just for clarity, in your original post you said it doesn't resume when you hit Cancel. I don't believe it is supposed to resume after you cancel it out. That is the purpose of the cancel, to cancel the settings. If you want to resume, don't cancel.
Glad its working for you now.

I have not. But the dealer has confirmed software issues related to the virtual assistant with several customers. So I'm assuming it's software. But so far, so good - except for virtual assistant, and also USB port not working.
