Gas or exhaust smell.... what could it be?

Might as well. I don't know if they have to come off to get to that part but it's right there anyway. No honest mechanic should charge you much more to knock that out too.

Can someone make some sense of these readings? Anything out of place ?

Random thought, YMMV. The engine is running lean, which can be seen as all the negative numbers for short-term and long-term fuel mix. The ECU is subtracting fuel from the air/fuel mixture to lean it out. The ECU bases those decisions on the amount of air flow the MAF sensor is detecting & reporting. If the MAF sensor senses too little air, then the ECU cuts back on the amount of fuel to compensate; the ECU records that action as running lean (leaner on fuel than normal). On the other hand, when the vehicle is running rich, when it is ADDING (too much) fuel, then you can sometimes smell that extra fuel around the engine ... which is where your cabin air intake is coming from, those vents near the wiper blades. Put those two things together and IT MAY BE THAT YOUR MAF SENSOR IS DIRTY, causing it to send incorrect air flow data, in turn causing the engine run too lean or too rich. The MAF sensor sits behind the engine air filter and is really just a set of very thin wires .. and over time those wires can get covered with road crud that causes them to not sense air flow correctly. You can usually fix a dirty MAF sensor by removing it then spraying it down correctly with about 10 quick squirts from an aerosol can of MAF sensor cleaner that you can pick up at the auto parts store for about $10 then letting it dry out completely before you reinstall it.
Also I would verify that the vehicle had its 2009-01-16 Safety Recall 9LA performed, replacing the OEM fuel rails. The rails were sometimes corroding due to fuel with incorrect ethanol moisture content, causing pinholes that leaked fuel. I would think, since this is a formal safety recall, that there is no expiration date for having the work done. You can look up the (Lexus dealership) service history for the vehicle by its VIN number at the Lexus Drivers website.

Thank you so much for reaching out on this matter. Yes, I do take note of the prior dating in a thread when posting. And I sometimes post to an older and/or dormant thread, where even the OP might well no longer be interested in the discussion. I do so mostly to benefit any other Lexus owner whom might someday search (internally or externally) for information and/or assistance regarding the topic under discussion in the thread ... and so be led to it. As example, Google led ME to this thread the day I posted to it ... as I continue to search & read information about possible causes of a slight odor of gasoline I have occasionally noted around my 2007 LS460. (For the moment, I am simply attributing it to a failed gas cap and have replaced it in order to exclude it, but I am continuing to educate myself as to other possible root causes.) Personally, I have learned much useful information from reading the contributions of other Lexus colleagues contained in old and/or dormant threads. Again, my appreciation for your interest toward best focusing my efforts.
