The IS-F DownShifts to first gear at a stop if you don't touch the paddle. When in S Mode, my 11 IS350 isn't showing a reduction in gears (ex. 4, 3, 3, 1) unless I shift it.
Are you actually looking at the 'gear it is in' and not "what you have S mode set to"?
because 4 is the default for S mode, but it's not always what gear it's in at the time...it's what the max allowed gear is.
It's highly unlikely your car is taking off in 4th from a stop.
