I had this same problem. Mine used to shudder during idles and even when i was driving sometimes.

I took it to the dealer and they ran a bunch of tests. Apparently, this is all linked to the "engine crank nose start up" problem.

They replaced cam gears and valve springs i think.

No longer shudders during idles.... startups... or driving. I can give a full list of things they changed when I get my car back (in the dealer for recalls).

Took the car in for the service campaign and I mentioned the shaking. My SA said Lexus is aware of the problem but there's not a lot of complaints to warrant another campaign. He mentioned about replacing the cam gear as well. BTW, part(s) is a special order.

So for you guys who have this issue, make sure to tell the dealer or LEXUS. Have them order the parts first before you get an appointment.

My car is still in the dealer because they have to wait 2 to 3 days for the parts to come. I left my car Saturday at 11AM. Service campaign job done by Monday afternoon. SA advised me to leave the car so they could start fixing the "shaking problem" as soon as they receive the parts. Car will be ready by midweek next week per SA.

Hopefully they fix the problem.

Will let you guys know exactly what they replace/fix when I see the work invoice.

Really. Interesting I will have to mention this to my dealer since this has been happening to me as well on and off every now and then when at stop signs or red lights.
