Default Jolt/ Jerk when braking to a stop

I noticed that when I am braking I sometimes feel a slight jerk when coming to a stop. If I slowly brake and coast to a complete stop, the jerk happens right before the complete stop. If I am making aggressive stop/go," no coasting to a stop, sudden quick stops", then the jerk will happen right before I gas it.

Not sure if I clarified the situation but if anyone have the same problem, please let me know what could be the problem.

how old are the calipers? jack the car in the air and rotate the tires. see if the brakes are catching.

Default Violent Jerk when coming to a stop

I have a 2004 Lexus ES330 with 148K miles. In the morning on my way to work, when coming to a stop my car will violently jerk when I am slowing down. It does it between 20 and 0 mph. There are no lights that come on. I then press the gas and it will jerk again and then kick into gear and continue on. The first jerk is very hard though and it is scarey. Feels like someone slammed into the back of me. Does anyone know what this could be?

Damn that's crazy.. My brakes are starting to squeak and squeal a little bit, definitely need to get new pads.
