Default need help with overheating

I have a 2003 IS300 and it started overheating yesterday. Water seems to be boiling out of the back part of the engine on the top. Would this be a head gasket? or could it be something simpler. I'm hoping for the latter.

Check the heater hose that goes into the back of the head.
